Art by Monte Dolack, image is featured in the Art of Rock book on page 335 (plate 4.9).
Medium-thin glossy stock poster is in beautiful near mint (A-/B+) condition; aside from a touch of edge wear (a few stray dings) and a hint of curl to the paper, this is probably one of the finest examples out there.
